For added convenience, you can print out and complete our new-patient forms at home before visiting the dental office. Bring your completed forms with you (or fax them to us at 509.427.5711 prior to your visit) and make your first dental appointment a snap. If you have any problems with the forms, simply call our office and we will gladly send or fax them to you.
